Item Assessment of a computer-based Taenia solium health education tool ‘The Vicious Worm’ on knowledge uptake among professionals and their attitudes towards the program2015(Elsevier, 2016) Ertel, Rebekka Lund; Braae, Uffe Christian; Ngowi, Helena Aminiel; Johansen, Maria VangHealth education has been recognised as a specific intervention tool for control of Taenia solium taenio- sis/cysticercosis but evaluation of the efficacy of the tool remains. The aim of our study was to assess the effect of a computer-based T. solium health education tool ‘The Vicious Worm’ on knowledge uptake among professionals and investigate attitudes towards the program. The study was carried out between March and May 2014 in Mbeya Region, Tanzania, where T. solium is endemic. The study was a pre and post assessment of a health education tool based on questionnaire surveys and focus group discussions to investigate knowledge and attitudes. A total of 79 study subjects participated in the study including study subjects from both health- and agriculture sector. The health education consisted of 11⁄2 h indi- vidual practice with the computer program. The baseline questionnaire showed an overall knowledge on aspects of acquisition and transmission of T. solium infections (78%), porcine cysticercosis treatment (77%), human tapeworm in general (72%), neurocysticercosis in general (49%), and porcine cysticerco- sis diagnosis (48%). However, there was a lack of knowledge on acquisition of neurocysticercosis (15%), prevention of T. solium taeniosis/cysticercosis (28%), and relation between porcine cysticercosis, human cysticercosis, and taeniosis (32%). Overall, the study subject’s knowledge was significantly improved both immediately after (p = 0.001) and two weeks after (p < 0.001) the health education and knowledge regard- ing specific aspects was significantly improved in most aspects immediately after and two weeks after the health education. The focus group discussions showed positive attitudes towards the program and the study subjects found ‘The Vicious Worm’ efficient, simple, and appealing. The study revealed a good effect of ‘The Vicious Worm’ suggesting that it could be a useful health education tool, which should be further assessed and thereafter integrated in T. solium taeniosis/cysticercosis control.Item Prevalence of porcine cysticercosis and associated risk factors in smallholder pig production systems in Mbeya region, southern highlands of Tanzania(Elsevier, 2013-09-07) Komba, Erick V.G; Kimbi, Eliakunda C; Ngowi, Helena A; Kimera, Sharadhuli I; Mlangwa, James E.; Lekule, Faustin P.; Sikasunge, Chummy S.; Willingham III, Arve Lee; Johansen, Maria Vang; Thamsborg, Stig M.Porcine cysticercosis (PC) caused by the larval stage of a zoonotic tapeworm Taenia solium, is known to pose serious economic losses and public health risk among smallholder pig production communities. The present study was conducted to determine prevalence and associated risk factors for PC in smallholder pig production systems in Mbeya region, the major pig rearing region of Tanzania. A cross-sectional survey employing a random sample of 300 pig keepers from 30 villages of Mbozi and Mbeya Rural districts, Mbeya region were used to evaluate pig production systems and practices. Concurrently, 600 male and female pigs of different age categories were randomly selected and examined for PC using lingual examination method and antigen enzyme-linked immunosorbent assay (Ag- ELISA). The overall pig level PC prevalence in Mbozi district was 11.7% (95% CI = 8.5–15.8%) and 32% (95% CI: 27–37.5%) based on lingual examination and Ag-ELISA, respectively. In Mbeya Rural district, the prevalences were 6% (95% CI: 3.8–9.3%) and 30.7% (95% CI: 25.8–36.1%) by lingual examination and Ag-ELISA, respectively. In Mbozi district 46% of the households were found infected (one or more infected pigs) and the corresponding figure was 45% for Mbeya Rural district. The agreement between lingual examination and Ag-ELISA was poor ( < 0.40). There were no significant differences in the prevalence of PC in different sex categories of pigs. Significant risk factors associated with PC preva- lence were free roaming of pigs (OR = 2.1; 95% CI = 1.3–3.6; p = 0.006), past experience of porcine cysticercosis in the household (OR = 2.6; 95% CI = 1.5–4.8; p = 0.002), increased age of pig (OR = 1.9; 95% CI = 1.2–3.0), slatted raised floor in pig pen (OR = 8.4; 95% CI = 1.0–70.0), in-house origin of the pig (OR = 1.6; 95% CI = 1.1–2.5) and sourcing of water from rivers (OR = 3.1; 95% CI = 1.6–6.3; p < 0.001) and ponds (OR = 5.0; 95% CI = 1.2–21.7; p = 0.031). This study has clearly revealed a high sero-prevalence of PC in the study area, which imposes a major economical and public health burden to the smallholder pig farmers. The study also points to a number of important risk factors in smallholder pig management that may be addressed (e.g. confinement, quality of pens and water sources) in future interventions and educational campaigns for control of T. solium.Item Productivity and parasitic infections of pigs kept under different management systems by smallholder farmers in Mbeya and Mbozi districts, Tanzania(Springer, 2015-05-05) Lipendele, Calvin Paul; Lekule, Faustine Paul; Mushi, Daniel Elias; Ngowi, Helena; Kimbi, Eliakunda Casmir; Mejer, Helena; Thamsborg, Stig Milan; Johansen, Maria VangAn on farm experiment was carried out to assess the effects of production systems on the performance of local pigs kept by smallholder farmers. Six villages from Mbeya and Mbozi districts, Tanzania were purposely selected based on the prominent pig production systems: free range, semi- confinement and total confinement. Fifteen pig keeping households were randomly selected from each village to par- ticipate in the study. A participatory rural appraisal and struc- tured questionnaire were used for collecting information from the households on pig production and reproduction perfor- mance. In addition, a total of 180 weaner pigs, 2–3 months old, were purchased and randomly allocated to the 90 partic- ipating households. The pigs were subjected to three produc- tion systems: free range (M1), confinement with local diet (M2) and confinement with a compounded diet and anthel- mintic treatment (M3). The anthelmintic treatment (piperazine citrate) was administered at 1 g per kg body weight. Faecal and blood samples were collected at month three of the exper- iment to assess the burden of intestinal helminths and sero- prevalence of Taenia solium cysticercosis, respectively. Sows kept under free range system were reported to have smaller litter size both at farrowing and at weaning compared to those kept under confinement. The experiment showed pigs under M3 had higher (P<0.05) liveweight gains (136 g/day) com- pared to pigs in M2 (73 g/day) and M1 (68 g/day). In addition, pigs in M3 had higher body length and heart girth size with the feed to gain ratio of 8.5. Free range pigs tended to have lower faecal egg counts for most worm species compared to perma- nently confined pigs. Sero-prevalence of Taenia solium cysti- cercosis was 26 %, with village prevalence ranging from 8 to 52 %. Although pigs kept in M3 performed better than the rest, the compounded feed was too expensive for the farmers to afford. Locally available feed types combined with vitamin and mineral supplements may be a more sustainable option.
