Item A comparative study of the sero-prevalence of peste des petits ruminants virus among districts of different Agro-ecological Zones in Tanzania(East African Journal of Science, Technology and Innovation, 2020) Mdetele, D.; Seth, M.; Kabululu, M.; Misinzo, G.; Komba, E.Peste des petits Ruminants (PPR), a disease affecting sheep and goats, was confirmed in Tanzania in the year 2018. Since then the disease has continued to spread into different districts, causing significant socio-economic losses to livestock keepers. This study aimed at determining the sero-prevalence of PPR in 32 districts from the coastal, semi-arid and plateau ecological zones, respectively. Sera samples were collected from sheep and goats, and analysed by competitive Enzyme Linked Immunosorbent Assay (c-ELISA). Findings indicated that six (18.8%) districts had very high PPR sero-prevalence of which four (66.7%; Chamwino, Kondoa, Mvomero and Kilosa) belong to the semi-arid ecological zone and two (33.3%; Bagamoyo and Mkuranga) to the coastal ecological zone. Three districts (9.4%) had high PPR sero-prevalence, all from the semi-arid ecological zone. Twelve districts had low PPR sero- prevalence of which two (16.7%) were from semi-arid, one (8.3%) from coastal and nine (75.0%) from plateau ecological zones. A zero PPR sero-prevalence was recorded in three districts and eight districts from semi-arid and plateau ecological zones, respectively. There was a statistically significant difference in sero-positivity between the different ecological zones, χ2 (2) = 9.121, p = 0.010, with a mean rank sero-positivity of 24.7% for coastal zone, 12.0% for plateau and 20.8% for semi-arid zone. Post hoc pairwise comparison with Bonferroni correction for multiple tests showed a statistically significant difference between plateau and semi-arid zones (p = 0.032). Although the coastal zone had a higher mean rank positivity than the plateau zone, the difference was not statistically significant (p = 0.083). The study suggests a zonal predisposition of PPR sero-prevalence with districts in the semi-arid and coastal zones having significantly higher values compared to those in the plateau ecological zones. Efforts for control of the disease need to concentrate in those two high risk ecological zones.Item Ecological and Epidemiological Findings Associated with Zoonotic Rabies Outbreaks and Control in Moshi, Tanzania, 2017–2018(International Journal Environmental Research and Public Health, 2019-08-07) Killewo, Japhet; Mtui-Malamsha, N.; Sallu, R.; Mahiti, G. O.Approximately 1500 people die annually due to rabies in the United Republic of Tanzania. Moshi, in the Kilimanjaro Region, reported sporadic cases of human rabies between 2017 and 2018. In response and following a One Health approach, we implemented surveillance, monitoring, as well as a mass vaccinations of domestic pets concurrently in >150 villages, achieving a 74.5% vaccination coverage (n = 29, 885 dogs and cats) by September 2018. As of April 2019, no single human or animal case has been recorded. We have observed a disparity between awareness and knowledge levels of community members on rabies epidemiology. Self-adherence to protective rabies vaccination in animals was poor due to the challenges of costs and distances to vaccination centers, among others. Incidence of dog bites was high and only a fraction (65%) of dog bite victims (humans) received post-exposure prophylaxis. A high proportion of unvaccinated dogs and cats and the relative intense interactions with wild dog species at interfaces were the risk factors for seropositivity to rabies virus infection in dogs. A percentage of the previously vaccinated dogs remained unimmunized and some unvaccinated dogs were seropositive. Evidence of community engagement and multi-coordinated implementation of One Health in Moshi serves as an example of best practice in tackling zoonotic diseases using multi-level government e orts. The district-level establishment of the One Health rapid response team (OHRRT), implementation of a carefully structured routine vaccination campaign, improved health education, and the implementation of barriers between domestic animals and wildlife at the interfaces are necessary to reduce the burden of rabies in Moshi and communities with similar profiles.Item Prevalence of Brucella infection and associated risk factors among children in livestock keeping communities of Morogoro, Tanzania(East African Journal of Science, Technology and Innovation, 2022) Hildegalda, C.; Komba, E.; Kazwala, R.; Mathew, ColethaBrucellosis is among the most common bacterial zoonotic diseases worldwide. It is estimated that up to 500,000 new human cases occur each year. Brucellosis is a priority zoonosis in most East African countries. In Tanzania, the disease is one of six priority zoonoses, however, little disease burden information is available especially in children. Children are of particular interest especially in pastoral communities due to increased risk of exposure in their routine activities associated with handling of livestock and their products. This study investigated brucellosis exposure status and associated risk factors among non-febrile children in pastoral communities. A cross-sectional design was used involving a total of 361 non-febrile children of six months to 14 years old, randomly selected from households and schools. Risk practices were evaluated using focus group discussions among selected children and thematic analysis was performed. Serum samples were collected and analyzed using Rose Bengal Test (RBT) and indirect Enzyme-Linked Immunosorbent Assay (iELISA) in parallel. The overall seroprevalence of Brucella infection was 3.3% (95%CI, 2%-5%) determined by both RBT and iELISA. Risk practices observed and cited by children comprised of direct and indirect contact with animals and their products. These include: consumption of raw milk and their products, unhygienic handling of animals and their waste especially assisting cows during parturition. Girls/female respondents were more likely to test brucella positive than boys (male) respondents (2.23 OR, 95%CI 66%-754%). Brucellosis among non-febrile children in the study area was present at a relatively low prevalence. Risk practices for transmission of this zoonosis were also identified which included consumption of raw milk, contact with animals, their products and discharges including abortion materials which represent the basis for increased pathogen circulation in the area. General education on the epidemiology of brucellosis and prevention of infection spread should be provided to the community including children.
